Student Release Forms

Students must complete BOTH South Texas Release Forms AND Trinity Pines Release Forms.

Medication forms only need to be completed if your student has medication.

All medication will need to be turned into the camp nurse at check-in. Meds should be in a Ziploc bag with the medication form in front.

All prescription medications must be in the original, labeled containers.​​

Adult Release Forms

Adults need to complete the Trinity Pines Release Forms (Page 1 & 2)

Medication forms only need to be completed if an adult has medication.

All medication will need to follow the same guidelines laid out for student medications.

EACH CHURCH: fill out one copy of Page 3 (certification form) and Page 4 (transportation form)

All sponsors and any students over the age of 18 must be listed on page 3. These people must have a completed Ministry Safe course AND have a completed background check on file with your local church.​​​

Ministry Safe Certificates
ALL people attending for your church that are 18 years of age and older (if they turn 18 before or during camp, they must be included) will be required to complete a MINISTRY SAFE certification and background check which must be turned in BEFORE coming to camp by emailing turkee@marotzcleaning.com.
